1) Who we are & what we do
CGM QuickCert provides same-day certification and NDSS submission of completed CGM Access Forms, and (if purchased) telehealth consults for setup guidance. We do not supply devices or guarantee stock/supplier timeframes.
2) Using our services
By paying for a service you confirm you’re NDSS-registered (or engaging us to complete/update registration) and you provide information that is true and complete. Services are for people in Australia.
3) Declarations (you acknowledge & agree)
These mirror the NDSS CGM Access Form – Clause 23 points communicated by the certifier:
Telehealth first: You will not apply/use any CGM device before your telehealth consultation, which initiates optimal use.
Starter kit delivery (if needed): If a starter kit is arranged, you consent to your name, address and phone being shared with the supplier solely to deliver the kit.
Internet & follow-up: You understand suitable internet access is needed to upload/download device data and to complete follow-up telehealth for setup/optimisation.
Privacy & NDSS use: You agree to the collection, use and disclosure of your information for NDSS purposes as outlined in NDSS forms and our Privacy Policy.
Own use only: CGM products accessed under the NDSS are for your use only.
Accuracy: Information you provide to us is true and complete.
4) Device choice & informed consent
Your device selection is made with your health professional. You provide informed consent for the specific device chosen for your use.
5) Payments, receipts & records
Payments are processed by Stripe. You’ll receive a receipt by email. We keep a record of your acceptance of these Terms & Declarations and any confirmations you provide during checkout (for audit/compliance).
6) Privacy
See our Privacy Policy at /privacy (required by Stripe). It explains what we collect, how we use it, disclosures (e.g., NDSS, suppliers for delivery), security, retention, and your rights (OAIC).

CGM QuickCert (“we”, “us”) provides NDSS CGM certification and related telehealth services in Australia. Contact: [email protected]
What personal information we collect
Identity & contact: name, date of birth, email, phone, address.
NDSS/eligibility: NDSS number, concession status, clinician letters.
Health information: diabetes type/management details provided in forms/telehealth.
Payment details: processed by Stripe; we do not store full card numbers.
Technical data: device/browser info, cookies, analytics, advertising pixels (e.g., Meta Pixel) if used.
How we collect it
Information you provide by email, website forms, telehealth, and uploaded PDFs.
From third-party tools we use to deliver services (e.g., Stripe, Google, Zapier) where necessary.
Why we use it (purposes)
To review eligibility, certify CGM forms, and submit via the NDSS Health Professional Portal.
To arrange starter kit delivery (if applicable).
To provide telehealth, customer support, billing, receipts, and compliance/audit records.
To operate, secure, and improve our website and services, and (if enabled) perform analytics/remarketing.
Disclosure of personal information (who we share with)
NDSS/Diabetes Australia to process CGM access forms.
Device suppliers (name, address, phone) only when a starter kit delivery is requested/required.

Method of disclosure
We disclose electronically via secure transmissions/portals (e.g., NDSS portal), encrypted connections (TLS), and secure APIs to named providers for the purposes above.
Security
We apply reasonable technical and organisational measures: TLS encryption in transit, encryption-at-rest by major cloud providers, access controls with least-privilege and multi-factor authentication, logging and file access review, and staff confidentiality. No method is 100% secure; we continuously improve safeguards.
Retention
We retain records for as long as needed to deliver services and as required by Australian healthcare and business record-keeping obligations (often up to 7 years after last contact, and longer for minors), then delete or de-identify them.
Your rights
